Through various testing, I have compiled a large list of information for you to view. I hope you will find this informative and something you can use to your advantage in your game. Now, after this rather short intro, we will firstly move onto damage mechanics.
Does health and happiness affect your damage output?
The answer is no. HOWEVER, it does affect your swing speed.
This is damage done to a blockhead wearing no armor when hit by 5 swings of a titanium sword by a blockhead with maximum stats:

Next, we will compare armor.
Armor defense stats.
No armor vs 5 titanium sword attacks:
-did quite poorly, as expected.
Tin armor vs 5 titanium sword attacks:
-Im quite surprised tin armor did this well. Seems like a ~50% damage reduction?
Iron armor vs 5 titanium sword attacks:
-Again, a bit unexpected.
Cant get ice armor yet since this is done in a survival world. Will add later though. I just need one chestplate.
Carbon fiber armor vs 5 titanium sword attacks:
-Jetpacks have the same protection as a carbon fiber chestplate. Tested.
Titanium armor vs 5 titanium sword attacks:
-exactly the same as carbon fiber.
In short, the best armor is carbon fiber and titanium.
Secondly, we have the damage output of every single tool and weapon.

The test was done with a blockhead on the death bar every time.
0 - 4% Health filled.
WORM = ~0.74% (134 worms required to fill health bar)

5 - 9% Health filled.

RAW DODO MEAT = 6.667% (15 raw dodo meat required to fill health bar)
10 - 19% Health filled.
RAW FISH = 10%
MILK = 10% -Tin bucket not included
COOKED DODO MEAT = 12.5%
FLATBREAD = 14.29% (7 flatbread required to fill health bar)
COOKED FISH = 14.29% (7 cooked fish required to fill health bar)
20% Health filled.
BREAD = 20%
DODO STEW = 20%
FISH CURRY = 20%
33.34% Health filled.
PIZZA = 33.34%
RAINBOW CAKES = 33.34%
And that just about concludes it.
Quick summary of all essential knowledge:
Health and happiness do not affect your damage output, but they do affect swing speed.
Carbon fiber is the same as titanium armor. Jetpacks also offer the same amount of protection as a carbon fiber chestplate.
All non weapon tools deal the same damage, meaning a flint spade does the same amount of damage as an iron axe.
Pizza is the most effective way of restoring your health, as it heals the same amount of health as rainbow cakes and is easier to make.
